Attention focus alters physical skin inflammation response
A study found that focusing attention on a skin prick test significantly reduces the size of the resulting inflammatory welt compared to being distracted by videos.

A study published in Nature Human Behaviour demonstrates that directing attention can physically alter the magnitude of a local inflammatory response in real-time. Researchers led by Nofar Mizrachi and Liron Rozenkrantz of Bar-Ilan University, alongside Menachem Rottem of Emek Medical Center, found that participants who focused on the skin prick produced smaller welts than those who were distracted. The experiment used a standardized histamine prick test to induce inflammation without the use of drugs or deceptive tactics.
Two sessions, three to five days apart
The research team utilized a histamine skin prick test, which creates a reaction that appears within minutes and peaks between fifteen and twenty minutes. Scientists measured the wheal, the raised bump, and the flare, the red halo, at six intervals over twenty minutes. They also recorded heart rate, skin conductance, skin temperature, and pulse. In the first experiment, 37 participants were instructed to either attend to the sensations in the pricked hand or watch neutral video clips.

Around nine in ten participants experienced a stronger reaction when they were distracted. The difference at the twenty-minute peak was considered large by standard conventions with a Cohen's d of 1.30. Significant differences began as early as the three-minute mark and grew over time.
To ensure the visual task itself did not bias the results, a second experiment involved 20 new participants. These individuals viewed 37 abstract shapes. In one condition, a shape change signaled them to return attention to the arm; in the other, they judged if the shapes were real. In this trial, reactions were approximately 1.6 times smaller under inward attention. Interestingly, participants reported feeling the itching and burning as stronger while they were focusing on the arm, even as the physical reaction size shrank.
A smaller welt is not automatically a better one
While smaller welts usually suggest better regulated immune activity, the authors note that inflammation is a protective process where size depends on the context. In an infection, blunting the response could potentially weaken the body's defenses. The study scope was limited to healthy volunteers, mostly women in their mid-twenties, recruited between February 2024 and July 2025. Because no molecular or cellular mediators were measured, the link between attention and the skin reaction remains an inference based on the observed patterns.
The authors noted that certain recovery analyses were exploratory and not part of the pre-registration. In the first experiment, nearly 90 percent of focused participants had stable or shrinking welts between fifteen and twenty minutes, compared to 46 percent under distraction. This recovery trend did not reach significance in the second experiment likely due to the smaller sample size.
Three sympathetic measures flat, one vagal index up
Skin conductance, heart rate, skin temperature, and self-rated anxiety showed no significant difference between the two conditions. However, heart rate variability was higher during inward attention. This metric serves as an indirect index of vagal activity, which the authors suggest points toward a parasympathetic route rather than a stress-based one. The study did not measure cortisol or other hormonal factors.
To test if the sensations themselves were the cause, 17 returning participants used a 5 percent lidocaine cream before the test. While the numbing reduced the flare, it did not eliminate the wheal. Under numbing, the wheal averaged 4.0 mm, which was still smaller than the 4.9 mm observed under distraction.
